Membership Application

Interested parties who are suffering from cancer or chronic treatment are welcome to join our association. Application form can be collected from our instructors in each venue or downloaded from this website. Please complete the application form together with 2 recent photos and relevant medical proof (e.g. Doctor appointment slip), plus the one-off membership fee of HK$500.00 and pass to our instructors.

For applicant who is currently eligible for any assistance under the Comprehensive Social Security Assistance (CSSA) Scheme, organised by the Social Welfare Department (HK), he/she is also entitled to a 50% discount off the HK$500 membership fee given all relevant documents are submitted together with the membership application form. If the application is not completed within 3 months by submitting all required documents, e.g. medical proof and photos, the membership will be cancelled and all fees paid will not be refunded.

You may choose one of the following payment methods:

1. Cheque:
  Addressed to "International Guolin Qigong Culture Research Association Ltd."
Pass the cheque, the completed application form and two recent photos to our instructors or designated person.
2. Deposit to Bank:
  Bank-in membership fee directly to our Bank of China account (account no. 012-581-1-022933-7). Applicant must present the original copy of bank-in slip together with the completed application form & two recent photos to our instructors or designated person.
3. Cash:
  Pass the money, the completed application form and 2 recent photos to our instructors or designated person in each venue.

*If applicant cannot present the original copy of bank-in-slip or provide insufficient/false information in the application form, it is subject to the final discretion of our Association on his/her membership approval.


Donation

Our Association has been approved as a charitable institution which is exempted from tax under S.88 of the Inland Revenue Ordinance. (Tax Exemption Charity no: 91/9354)

Doner is entitled to tax deduction for his/her aggregate donation (excluding membership fee) of HK$100 or above in a year of assessment.
